Joel T Levis, MD, PhD, FACEP, FAAEM, is a Senior Emergency Physician at the Santa Clara Medical Center, and Clinical Instructor of Emergency Medicine (Surgery) at Stanford University. He is the Medical Director for the Foothill College Paramedic Program in Los Altos, CA. E-mail: [email protected]. Danny L Sam, MD, FACP, is a Senior Internal Medicine Physician, Director of Graduate Medical Education, and Director of the Internal Medicine Residency Program at the Santa Clara Medical Center in CA. E-mail: [email protected]. Phlegmasia cerulea dolens (PCD) is a rare form of massive venous thrombosis of the lower extremities associated with a high degree of morbidity including venous gangrene, compartment syndrome, and arterial compromise. Risk factors for PCD include malignancy, immobility, heart failure, heparin-induced thrombocytopenia, prothrombin states (eg, antiphospholipid syndrome), pregnancy, surgery and venous instrumentation (eg, placement of central venous catheters and inferior vena cava filters). Clinically, PCD is characterized by sudden pain, swelling, purple ecchymosis, and arterial ischemia with loss of distal pulses. Edema develops rapidly, and the skin of the affected extremity is usually tense, firm, and tender to palpation. Doppler ultrasound of the affected extremity should be used to confirm the diagnosis of PCD, and initial treatment includes bed rest, elevation of the affected limb, and systemic anticoagulation with heparin. Catheter-directed thrombolysis and venous thrombectomy should also be considered as early treatment options for PCD. v

Surgical Thrombectomy for Phlegmasia Cerulea Dolens

Phlegmasia cerulea dolens (PCD) is a medical emergency that can lead to venous gangrene of the lower extremity. Early diagnosis and prompt treatment is crucial for limb salvage. There are two treatment options (endovascular or surgical). In the endovascular era, catheter-directed thrombolysis is the treatment of choice to achieve venous outflow. However, surgical thrombectomy is indicated in ce...

Phlegmasia cerulea dolens in patient with systemic lupus erythematosus in the remote postpartum period.

Vascular manifestations are not rare in systemic lupus erythematosus and, in most cases, are associated with antiphospholipid antibodies. Phlegmasia cerulea dolens is an unusual and severe complication of deep venous thrombosis of the lower limbs, which has a high mortality rate. In the literature, only two cases of phlegmasia cerulea dolens associated with primary antiphospholipid syndrome hav...

Phlegmasia cerulea dolens in a long distance driver.

Phlegmasia cerulea dolens (PCDs) is a rare and serious complication of massive deep venous thrombosis of the lower extremities, which has a high mortality rate. It is characterized by the sudden onset of severe leg pain, massive edema, cyanosis, venous gangrene, compartment syndrome and arterial compromise, often followed by pulmonary embolism and death. We report one case of a long distance dr...

Phlegmasia Cerulea Dolens after Coronary Artery Bypass Surgery: What Should We Know

Phlegmasia cerulea dolens (PCD) is one of the most critical disorders of acute deep vein thrombosis in that it can cause permanent disability secondary to the compartment syndrome. Although several etiological factors have been proposed, PCD after coronary artery bypass surgery is extremely rare and its definitive pathophysiology is still under debate. We herein present a case of PCD that resul...

Simultaneous arterial and venous ultrasound-assisted thrombolysis for phlegmasia cerulea dolens.

Phlegmasia cerulea dolens is a rare condition in which an extensive deep venous thrombus can partially or completely occlude venous outflow from the affected extremity. Clinical presentation is typically characterized by extremity edema, cyanosis, and pain. This condition is associated with a high rate of extremity amputation and mortality. Although numerous therapies have been described, there...
